Plan Type Field

The Plan Type Field which includes a drop-down menu of Superior or EyeMed only appears on VW.com; it does not appear on PMS's or Co-brand sites. This is field is used as a reference for the labs and is only available for Essilor suppliers. Once a Plan Type is selected, the Authorization Number field becomes a required field. The authorization number for the Plan Type, Envolve, is the member's ID number.

9/30/25 UPDATE: Envolve is now the only provide listed on the Plan Type field. Eyemed was removed by request of Essilor.
